What to Bring
If you are a new patient at Smith Medical Clinic, please bring:
- Photo ID
- Current medications
- Proof of address
- Proof of income (tax returns or last two pay stubs)
While we would love to serve everyone and our care is without cost, we cannot see you without this information.
